We’re so glad you’ve decided to join this community. Here’s some info to help you get started.

Finishing signup:

  1. Please download the SigmaMD app to your phone. You should have a text and an email with your unique code that you can use to link the app to your chart.

  2. Schedule your first appointment. From the SigmaMD home page, choose "Schedule a visit with your clinician" and then choose the “New Patient Visit” (for families of youth seeking gender-affirming care please choose the "New Adolescent GAC Visit" appointment type). Please email Dr. Mirer if you need to be seen sooner than the next available appointment (for example you are feeling ill or running out of medicine).

  3. Check your email for three messages with links to digital forms for you to complete. The forms will take most folks about 10-20 minutes to complete.

Before your first visit:

  1. Write down or record on your phone a list of questions or issues to discuss with Dr. Mirer.

  2. Bring all your medicines with you to your appointment, including vitamins, herbal preparations, and over the counter medicines.

  3. If you have medical records from anywhere other than Aurora, Froedtert, or Children’s Hospital of Wisconsin, please request access to them ahead of time. You can usually get access online or through apps such as MyChart. You can either print them out or show them to Dr. Mirer on your phone. Please note that many hospitals have increased privacy protections for records pertaining to gender-affirming care, and you may need to place a special request through your old clinic. 

How to get in touch:

During business hours: The easiest/best/fastest way to get in touch is through a secure message in the SigmaMD app. Email is also a good way. Voicemail is a last resort. Dr. Mirer usually cannot answer the phone during business hours, because she is seeing other patients. She will make every effort to respond within 24 hours. Please wait until the end of the next business day before repeating your message. Please note that due to the recent influx of new patients Dr. Mirer is currently taking longer than usual to respond to messages.

After hours: Dr. Mirer does answer calls after hours, but please only call after hours if it’s the kind of call that should wake her up overnight, delay family dinner, or interrupt an episode of Heated Rivalry (see guide).

Appointment requests

You can send a message to request any type of appointment. Urgent appointments can also be self-scheduled through the SigmaMD app. Dr. Mirer may have additional appointments available that do not show up in the app. Dr. Mirer can usually get people in for urgent visits by the end of the next business day. 

Prescription refills:

From the pharmacy:

  • Please check your prescription label to see if you have any refills remaining before requesting a new order.

  • Please request refill orders 1 week before you will run out of pills, to leave time for things to go wrong. 

  • You can request a new order to the pharmacy by sending Dr. Mirer a secure message or email.

  • Please make sure to specify which pharmacy to send it to. If it’s a new pharmacy for you, please provide the address.

Meds dispensed by Dr. Mirer:

  • Please send your request by secure message or email

  • Dr. Mirer will let you know when your prescription is ready, and will leave it for you in the purple med pickup locker next to the reception desk at Presence on the Monday-Thursday of your choosing.

  • You will have until the end of the day (6:00) to pick up your medicine.

  • Each patient has their own unique code for the lock.

After-Hours Communication Guidelines. Regular hours are 9 AM to 6 PM,. Monday through Thursday. For emergencies, call 911 or go to the nearest ER. For non-emergencies, send Dr. Mirer a non-urgent message. Call Dr. Mirer if you're not sure which.
